When requesting a medication refill for a controlled substance:

• Ritalin/methylphenidate
  
• Dexadrine/Dextrostat
  
• Metadate
  
• Concerta
  
• Focalin

• Your pharmacy requires a written script in order to fill a prescription for controlled substances.
  
• Please contact our office at 763-550-9005 to request a script for your prescription
  
• Please allow 2 business days for your request to be processed
  
• Due to staffing constraints, medication refill requests submitted after 3:30pm will be considered received on the next business day
• Requested scripts that have been processed may be picked up at our office. Please call the office (763-550-9005) to verify a pick-up time as our staff hours may vary.

When requesting a medication refill for non-controlled substances (all medications that are not listed above):

• Contact your pharmacy and ask them to fax our clinic a refill request at 763-559-2118
  
• Please allow 2 business days for your request to be processed
  
• Due to staffing constraints, medication refill requests submitted after 3:30pm will be considered received on the next business day
